Drug resistance is caused by mutations in the bacteria's genome. Due to natural selection, the bacteria that become resistant to the drugs reproduce and create a slightly different species that is not affected by the drug.

This article from CNN talks about some of the reasons as to why more and more diseases are becoming antibiotic-resistant. One of these reasons is improper use, which causes the diseases to develop more quickly. Use of antibiotics on farm animals also causes the bacteria to grow and change around animals that we eat (pigs, chickens, etc.). It also mentions how most major diseases, like TB, are becoming resistant to medicines that used to be very effective in their treatment.
(Note: I felt like doing two of the choices)
Scientists will probably be hard pressed to find new cures for these diseases, and hasty work could produce medicines that are not as effective as previous ones.

i have no idea at all what is causing this. im no professional. id probably say some mutation of some sort happened along the way.
